Chaos Extended Quad

What were they caused by?

The injudicious use of a flat head screwdriver to get the two halves to line up. I don't know all the specifics about this one, since it was my buddies. Some combination of his gun and the quad ended up with that corners screw holes not perfectly aligning. This is the unit mounted on the Saiga with the Propoint in my gallery in the shot from the range when we went trap shooting.
